New Patient Registration
ONCE THE APPLICATION FOR NEW PATIENT REGISTRATION IS SUBMITTED, IT TAKES ABOUT 7-10 WORKING DAYS TO GET REGISTERED. WE WILL NOTIFY ONCE YOU ARE REGISTERED ON OUR SYSTEM.
If you currently reside within our Practice Boundary you can begin your registration as a 'New Patient' by completing the GMS1 registration form AND the New Patient Health Questionnaire.

Registering with our Practice - completing the GMS1 patient registration form
When filling in the registration form it is important that the form is fully completed to comply with the Health Authority requirements:
- Previous surname (if any)
- Town and country of birth
- Home address with postcode
- Telephone number
- Previous address
- Name and address of previous GP
- If appropriate, date of arrival in the UK

The surgery list is open to any eligible U.K resident falling within the catchment area. If you have an NHS record card and proof of current address, please bring this with you when you register.
All new patients registering with the practice are invited to have a new patient health check to identify specific health needs.
Please be ready to tell us details of your past illnesses, medication, hospital admissions and any other relevant information.
Medical treatment is available from the date of registration. Please contact reception for further information.

Change of Details
Patients are asked to notify the surgery as soon as possible of any change of name, address or telephone number; not forgetting to indicate all the persons involved in this change. In an emergency, this could be absolutely vital.
